ORTIZ -- John Matthew Ortiz of Santa Fe, New Mexico, resident of Plano, Texas, loving son and brother passed away May 30, 2004. His brother the late Michael P. Ortiz M.D. preceded him in death. John is survived by his mother and father, Frank J. Ortiz and Lucille N. Ortiz; brothers, Frank Ortiz III and wife, Rose, Mark A. Ortiz and wife, Yvonne of Albuquerque, NM; sisters, Anne Ortiz M.D., Linda Lashen and husband, Woody of New York he is also survived by ten nieces and nephews. He is also survived by his special friend, Luchie Ibarra of Plano, TX. A Rosary will be recited on Friday, June 4, 2004 at 7:00 p.m. and a Mass of Resurrection at 11:00 a.m. on Saturday, June 5, 2004 at Prince of Peace Catholic Church, 12500 Carmel Ave. N.E., Albuquerque, NM. John graduated from the University of Notre Dame in 1984, where he received a BA Degree in Government and from the University of Texas at Austin in 1986 where he received a Master of Public Affairs Degree. John entered Federal service with the U.S. Commerce Department, Census Bureau in October of 1986, where he was employed for over 17 years in both Washington D.C. and Dallas, TX. In November 2001 he received the U.S. Department of Commerce Silver Medal Award, presented in Washington D.C., for Meritorious Federal Service. John received numerous citations and awards from the U.S. Census Bureau and other U.S. government agencies including several Hispanic organizations for his selfless efforts to promote the mission of the U.S. Census Bureau throughout the Southwest region. John volunteered with both religious and academic organizations, including the Notre Dame Club of Dallas where he served as an officer. He also served as past president of the St. Rita's Young Singles (SRYS) in Dallas. John also sang in the choirs at both St. Rita's in Dallas and St. Elizabeth Ann Seton Catholic Church in Plano, TX. In lieu of flowers, the family asks that you donate to any of John's favorite charities as follows; St. Elizabeth Anne Seton Catholic Church; 2700 W. Spring Creek Parkway, Plano, TX ; Maryknoll Missions, PO Box 304 Maryknoll, NY 10545-0304 Care of Fr. Leo Shea; American Heart Association 1280 S. Parker Rd. Denver Co. 80231-2100. The Ortiz family extends its deepest gratitude to all of John's co-workers, friends and family who have conveyed their prayers and love during this time of loss. Pallbearers include Daniel Provencio, Mark Stephen Ortiz, Woody Lashen, Louis Ortega, John Patrick Ortega, Jerry Rattenbury.
