SANTIAGO "SANTI" RAFAEL Our beloved son, father and brother, born on September 3, 1991, departed us and became one with our Heavenly Lord on Thursday, June 21, 2012. Santiago blessed us all with his kind soul and unconditional love! He will be greatly missed and always with us! He was preceded in death by his grandfather, Ray Hernandez and his nana Corrina. He is survived by his loving parents, Rachael Hernandez and Hugo Chavez; his son, Santiago M. Chavez; sisters, Amanda, Marissa and Ameliana Chavez; brothers, Elijah Chavez and Steve "Sport" Portillo; uncle, George Hernandez; grandparents, Kathy and James Wujick, grandma and grandpa Chavez; great-grandmother, Josie Martinez; Joee Ruiz; life long best friends, Eddie Chavez and Patrick Montoya; and many dear aunts, uncles, cousins and friends. A Memorial Service and Celebration of Life reception will be held for Santiago on Friday, June 29th at 6:30 pm at The Church at 414 Charleston Street SE, Albuquerque, NM 87108.
